Manual Therapy

Manual therapy is the hands-on part of physiotherapy: techniques applied directly to joints and soft tissue to restore movement and reduce pain. On its own it brings relief; combined with exercise it brings lasting change. Fzt. Kemran Hafız therefore never uses it as a stand-alone treatment.

Manual therapy applied to the wrist and forearm

When is it needed?

  • Joint stiffness and restricted movement accompanying a neurological condition
  • Tightness from increased muscle tone (stiffness that's confused with spasticity)
  • Stiffness in the shoulder, hip or ankle after prolonged inactivity
  • Being unable to move past a certain angle during exercise
  • Asymmetric tightness caused by one-sided use

What we do

  • Determining by neurological exam whether restriction comes from the muscle or the joint
  • Targeted joint mobilisation
  • Locking in the gained range with active exercise in the same session
  • Soft-tissue work in areas with increased tone
  • Regular re-measurement of range of motion

Manual Therapy in Bursa Nilüfer

Fzt. Kemran Hafız provides one-to-one care for manual therapy at the clinic in Bursa Nilüfer — in Fethiye Mahallesi, on Kelebek Sokak, at a central point of Nilüfer, opposite the Fethiye covered market.

It is a short trip by car or public transport from İhsaniye, Odunluk, Balat, Özlüce, Ataevler, 23 Nisan and the surrounding areas. Every session is one-to-one, assessment-based and tailored to you.

The stage that opens the door to exercise

In a joint that has been immobile for a long time, or where muscle tone has increased, the tissues shorten. As range of motion narrows, exercise itself becomes impossible — this is exactly where the process gets stuck.

Fzt. Kemran Hafız uses manual therapy in neurological programmes to untie this knot: the joint is opened by hand first, and the gain is locked in with active exercise in the same session. Manual therapy applied on its own doesn't give a lasting result in this picture; its job is to make the real work possible.

Why manual therapy first, then exercise?

In a neurological condition, if the restricted joint doesn't have the range needed for exercise, attempting it turns into strain and compensatory movement. Manual therapy earns that range by hand first; the gain is lost within a few days if it isn't reinforced with active exercise that same day.

That's why Fzt. Kemran Hafız plans manual therapy not as a session on its own, but as the opening step of the exercise programme.

How are increased tone and joint stiffness told apart?

Increased muscle tone (spasticity) and genuine joint stiffness are handled with different techniques; confusing the two sends the programme in the wrong direction. A neurological exam clarifies the source of the restriction and the choice of technique follows from it.

Which neurological conditions is it used for?

Post-stroke shoulder stiffness, joint contractures after prolonged bed rest, and asymmetric tightness from one-sided use are the most common areas. The aim is always the same: making exercise possible.

How is progress measured?

Range of motion is measured in degrees at regular intervals, and how much of that gain carries over into the exercise programme is tracked.

Who is it not used for?

Manual techniques are not used, or are modified, in active infection, a recent fracture, advanced osteoporosis, suspected tumour in the area, and certain vascular conditions. Medical history, medication and diagnoses are asked about in detail at the first session; if an unsuitable picture is found, the technique is changed or a referral is made.

Frequently asked questions

Does manual therapy reduce spasticity?

Manual therapy can soften increased tone temporarily; lasting management comes from correct positioning together with the whole exercise programme.

Is manual therapy applied at every session?

No; it's used only when a stiffness or tightness is restricting exercise, with the purpose of clearing the way for the exercise programme.

Is manual therapy enough on its own?

No; the range of motion gained doesn't last unless it's locked in with exercise in the same session. In a neurological condition, the exercise programme does the real work.
