ESWT — Shock Wave Therapy

ESWT (Extracorporeal Shock Wave Therapy) applies acoustic waves to tissue through a device. It is particularly favoured for long-standing tendon and tendon-insertion complaints. Fzt. Kemran Hafız does not offer ESWT as a stand-alone solution; loading and strengthening exercises stay at the centre of the programme so the effect of the treatment lasts.

ESWT for stubborn tendon and calcific pain

ESWT (extracorporeal shock wave therapy) is used particularly for tendon and calcific pain that has persisted for months despite exercise and manual therapy. Controlled pressure waves delivered to the tissue aim to restart the healing response. It is usually not a first step but a method used in suitable, stubborn cases.

Fzt. Kemran Hafız applies ESWT in Bursa Nilüfer always after an assessment and as part of an exercise programme — never as a stand-alone treatment. Because of his work in neurological rehabilitation, he also gives particular care to the limited but appropriate use of ESWT in selected cases such as post-stroke shoulder pain.

Which conditions is ESWT used for?

ESWT is most often chosen for stubborn (chronic) tendon and calcific pain. The main conditions Fzt. Kemran Hafız assesses for ESWT in Bursa Nilüfer are:

Foot and heel: heel spur, plantar fasciitis, Achilles tendinopathy. Elbow: tennis elbow (lateral epicondylitis), golfer's elbow (medial epicondylitis). Shoulder: rotator cuff tendinopathy, calcific tendinitis of the shoulder. Knee: patellar tendinopathy (jumper's knee).

Hip and surrounding area: hip tendinopathy, gluteal tendinopathy, greater trochanteric pain syndrome. Other muscle-tendon conditions: hamstring tendinopathy, chronic muscle and tendon pain, myofascial pain, trigger points within muscle, calcific tendinitis and tendon injuries. In selected cases, it may also be considered for chronic low-back and neck pain.

Not every condition on this list is automatically suitable for ESWT; which one, when and how many sessions is decided after assessment. You can reach the detailed pages for topics such as heel spur, tennis elbow and golfer's elbow from the related treatments section below.

How does it work, and when does it come in?

Shock waves create a controlled stimulus in the tissue; the aim is to restart a stalled healing process. ESWT is not a miracle on its own — the real change comes from exercises that gradually load the tendon. That is why ESWT is considered a step added to speed up the programme in conditions that are not progressing despite exercise.

The place of ESWT in neurological rehabilitation

After a stroke, shoulder pain and restriction in surrounding tissues can reduce participation in exercise. In these selected cases, Fzt. Kemran Hafız uses ESWT with a limited number of sessions to ease the pain barrier so the person can take part in their main movement and strength programme. Here too, ESWT sits alongside the programme, not on its own.

Who is it not suitable for?

ESWT may not be suitable during pregnancy, or where there is an active infection at the application site, a suspected tumour, or a bleeding disorder / use of blood thinners. For this reason suitability is always assessed first; where it is not appropriate, it is not applied and this is stated clearly.

Frequently asked questions

Which conditions is ESWT used for?

Most often for stubborn tendon and calcific pain such as heel spur, plantar fasciitis, Achilles tendinopathy, tennis/golfer's elbow, calcific tendinitis of the shoulder and patellar tendinopathy. Suitability is assessed for each condition.

Is ESWT a definitive cure?

No; ESWT is not a miracle method and the outcome depends on the condition. The real recovery comes from exercises that gradually load the tendon; ESWT is a step that supports this programme. No guarantee can be given.

How many sessions are needed?

It varies by condition and response; the number of sessions is kept limited and reassessed at each stage.

Is ESWT painful?

There may be a sensation of discomfort during application that varies by area; the intensity is adjusted to the person. Short-term tenderness afterwards is possible.

Other treatments

For heel pain that bites on the first step of the morning: shockwave, load management and a graded strengthening programme.

  • Tennis Elbow

For pain on the outer elbow that flares with gripping: a graded tendon-loading programme that rebuilds grip strength.

  • Golfer's Elbow

For inner elbow pain that flares with gripping and bending: a programme that raises tendon capacity step by step.
